You can request a survey by using the form to the right. Alternatively you can send us a separate email to survey@jensenassoc.com, or by calling us on Telephone +44 (0)208 106 4500. Calls are attended to 24 hours a day, 365 days a year.

 

Documents and information required

 

To the extent it is available, please forward the following details to us together with your request, which will be of assistance in the handling of the case and subsequent completion of reports:

 

  • Full address of the location where the survey is to be carried out

  • Contact name and telephone number for the person we should contact at the location to coordinate the survey

  • Copy of Bill of Lading, Waybill, CMR or any other transport document as relevant

  • Copy of invoice and packing list

  • Insurance certificate

  • Any temperature records available

Upon receiving your request we will proceed to arrange survey as required and send you a confirmation that forwarded documents and details have been received, along with a confirmation of the time agreed for the survey.

 

When our assistance is urgently required, we recommend that if you have not received a confirmation to an e-mail request within 30 minutes, please contact us directly by telephone to verify whether your e-mail has been delivered.
